The Physicians and Staff of Evergreen Women’s Health Center would like to welcome you to our clinic, which is located in the Evergreen Surgery & Physician Center. It is our goal to provide the best medical care possible and to meet your needs as our patient both physical and psychosocial. On our website, you will find answers to a few basic health care questions, information you should know about each of us not only as health care providers but as people, as well as a listing of our services. OFFICE HOURS Our office is open Monday through Friday, 7:30 AM to 5:00 PM. Evergreen Women’s Health Center is closed on holidays. Our receptionists are available to schedule an appointment for you beginning at 7:30 AM. After hours, our office number, 425-285-0060, is answered by our answering service, which will put you in touch with the doctor on call in an emergency. OBSTETRIC CALL SYSTEM In general, during office hours, each physician tries to deliver her own patients if she is working in the office. If your physician is not available, the physician on call will take care of you. Because of our commitment to providing the best possible care to our patients, our physicians rotate responsibility at night, on weekends, and on holidays. Under normal circumstances, you will usually be seen by your own physician from our group or one of our nurse practitioners. On weekends and weeknights, the on call physician will handle urgent requests. APPOINTMENT CANCELATIONS Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we will be happy to reschedule your appointment to a time that is more convenient for you. TELEPHONE CALLS In an effort to best serve all of our patients, we encourage you to have your medication refills and clinical questions resolved during normal business hours. This policy will allow us to better meet our patients’ urgent medical needs after hours. CONTACT TRIAGE To contact your physicians nurse during normal business hours, simply call us at our main number (425) 285-0060 and choose option 3 to leave a detailed voicemail message. Please include the spelling of your first and last name, date of birth and the phone number where you can be reached. Your call for medical advice will be routed to a nurse who will consult with your physician and call you back. This line is NOT for medical emergencies. If you are experiencing an emergency, please call 911. AFTER HOURS If you have a question or problem of an urgent nature after business hours, please call (425) 285-0060. You will be directed to the after-hours answering service. The physician on call will be notified of your message and return your call as soon as possible. The service is for urgent concerns; non-urgent issues and prescription refills should be addressed during regular business hours. PRESCRIPTION REFILLS The best method of having a prescription refilled is to have your pharmacist contact our office by phone at (425) 285-0060, Option 3, or by fax at (425) 285-0070. All prescription refills can take up to 72 hours to be refilled, for this reason, please make your requests at least 72 hours in advance of your anticipated need. By law, certain narcotic prescriptions cannot be phoned into a pharmacy. Please let us know in advance and a written prescription, if appropriate, can be provided for you by your physician. INSURANCE All insurance co-pays should be paid at the time of your visit. It is always a good idea to know your insurance benefits. Referrals from your primary care provider may be necessary to insure that you receive your highest benefit available. If you do not comply with the requirements of your health plan, you may be personally responsible for your bill. We accept most insurance plans as well as Mastercard, Visa, Discover and American Express. We will also allow payment plans for services if discussed with our billing department in advance. Our bookkeepers are available to assist you with any billing questions you may have. ONSITE LAB Our office has an onsite laboratory to draw blood and process urine tests. This enables our patients to complete their testing while still in our office. We also have the capability to obtain certain results the same day if necessary. Your physician or nurse will notify you with results by letter or by phone. MEDICAL RECORDS Your medical record is confidential. It will not be given to anyone without your written consent. DISABILITY FORMS Our office will be happy to complete your disability form if medically indicated. Please allow 5-7 business days for completion. If more than one form is needed for the patient or a family member of the patient, there is a clerical fee of $35 per form. |
